Excise-tax policies vary widely across States, with some States imposing taxes on prices (i.e., ad valorem taxes) and others levying excises on quantity or volume (i.e., specific taxes). All States impose a tax on beer; in addition, all license States also impose taxes on wine and spirits.3 In general, these State excise taxes are highest for distilled spirits. State excise taxes, for the most part, have followed the same patterns as Federal taxes, with only infrequent and modest increases that have resulted in substantial declines over time in the real values of these taxes. The degree to which the real value of the State taxes has dropped depends on the inflation rate and the latest tax rates imposed by a given State. More than 20 States have not raised their beer taxes for at least 20 years, and only about 10 States have raised them in the last decade.4 In some extreme cases, the deflated tax rates per drink have even declined to close to zero.

Policies
A review of these trends suggests just how important effective prevention and treatment can be. For example, in 1964 the Surgeon General issued a report that declared cigarette smoking as harmful to one’s health. The Surgeon General’s warning was then required to be placed on each pack of cigarettes. Undertaking the research to support the warning and requiring that the warning be advertised was a relatively inexpensive form of prevention that had a significant impact on reversing a more than 60-year trend in the per capita consumption of cigarettes. The introduction of the nicotine patch in 1991 significantly helped smokers reduce their consumption and cease their dependence.
An alcoholic who drinks a 12-pack a day consistently for a year spends over $3,000. This cost doesn’t include potential legal issues that can cost thousands more. It’s harder to estimate the price of illicit drug addiction, but it can be much higher. While trends generally indicate an increased interest and purchasing of premium-priced alcoholic beverages, there is evidence that younger consumers are particularly interested in trading up. Still, other data indicated that in 2023, 52% of consumers aged 18 to 34 felt that “drinking in moderation is bad for your health,” an increase from 34% reported in 2018 for this age group. Fifteen percent of this age group indicated that alcohol was “not too harmful/not at all harmful” to those who consume it.
Insurance plans and public programs with limited coverage of substance abuse treatment are likely to also have severe limits on mental health coverage. For example, Mast and colleagues (1999) replicated the analysis by Chaloupka and colleagues (1993) but used data obtained from 1984 to 1992, rather than from 1982 to 1988. These investigators failed to detect a significant relationship between tax rates and drinking-and-driving rates. Likewise, Dee (1999) and Young and Likens (2000) found that there was little relationship between beer taxes and motor-vehicle fatalities, and the significance of the relationship was very sensitive to the specification of regression models.
Likewise, Gallet (2007) showed that among 300 estimates, the median elasticities were −0.36, −0.70, and −0.68 for beer, wine, and spirits, respectively. The Community Preventive Services Task Force—an independent, nonfederal, volunteer body of public health and prevention experts—recommends several evidence-based strategies to reduce excessive alcohol use and related harms at the population level. These strategies can help communities create social and physical environments that encourage drinking less alcohol.
